Release of Information​

Before your initial intake visit, you will receive an email prompting you to check in. This email will sent one week before your scheduled appointment and will include all necessary intake paperwork and consent forms that must be completed at least 24 hours prior your visit. You will be able to ask questions and verify insurance during this check-in process as well.

Your provider may request records from any past or current to ensure they can offer you the best possible care. You can complete and submit a Release of Information (ROI) form here, before your first appointment. We encourage you to take a few minutes to complete your information electronically. Paper forms are also available and can be completed when you come to your clinic appointment.

Records
To make the most of your appointment time, it is helpful if you can send over your medical records as early as possible, before your first appointment. These records should include past psychiatric hospitalizations, recent visits to primary care or specialty care, any records from previous mental health treatment, psychological or neuropsychological evaluation reports, as well as any other records you may feel important for your provider to have to be able to better care for your child. This may include your child's Individualized Education Plan (IEP), if applicable.

You can contact previous or current care providers, and request that they fax records directly to Endless North Clinic at (763) 205-5834. If you have physical or digital copies of these records, after you have set up a portal account, then you may upload these records directly to your portal account, where they will be reviewed by your provider.

Any physical copies of records may also be brought with you to your clinic appointment, and these may be copied at that time.
Custody Documentation
Mental health providers must obtain appropriate consent for services. This entails having custody or guardianship documents for individuals who cannot consent independently such as vulnerable adults or minors. You will be asked about any custody arrangements prior to your first appointment. If you share custody with another parent, we will require a copy of your divorce decree, or other legal documentation that identifies legal and physical custody.

If legal custody is shared and either parent or legal guardian does not consent to treatment, or if custody documentation cannot be provided at least 24 hours prior to your first appointment, it may need to be canceled and/or rescheduled until this paperwork is obtained. ​
