Payments
We will always do our best to work with your insurance for coverage of your appointment. If there are any costs that you may be expected to pay outside of that, such as co-pay costs, we will communicate that with you. At this time, we can accept cash, check, and credit card payments. You may be asked to keep a card on file to cover outstanding payment balances. We offer payment options as follows: ​
In Person
You may bring payment with you to your in-office visits.
By Phone
You may submit your payment information over the phone, by calling the clinic at 763-284-1877, any time during our business hours.
In the Portal
Payment can also be made via the Patient Portal. Once logged in, you will see the option to submit a one-time payment. You may also keep your payment information on file for future payments.
By Check
Please make check payments out to Evernorth Clinic PLLC PO Box 48 Albertville, MN 55301. You may also mail any check payments to the same address.
Reduced Fee
Reduced fee services are available on a limited, case-by-case basis. Please speak with your provider for more details.
Good Faith Estimate
In Minnesota, you have the right to a Good Faith Estimate (GFE) for the total expected cost of non-emergency healthcare services, especially if you are uninsured or not using your insurance. Providers must give you this estimate in writing when you schedule services at least three business days in advance or upon request. If your final bill is $400 or more higher than your GFE, you have the right to dispute the bill. Be sure to save a copy of your good faith estimate.
